Emergency Information    A new emergency form must be filled out for each sport.

Safety  -  One for each sport - soon to be added.

Once you have completed the top 4 forms you won't have to do them again during the school year.

The emergency information and safety forms must be filled out for each sport your child participates in.

Forms may also be picked up in the main office.

Fee Information:

Sports user fee---------------------$30 for each sport your child participates in -

Student qualifying for free/reduced---$7

ASB card--------------------------$15

Student qualifying for free/reduced---$5

Yearbook--------------------------$20

ASB & Yearbook together---------$30

Student qualifying for free/reduced---$25
